These injection techniques introduce hyaluronic acid (HA) into or underneath the skin, and provide the physician 4 ways to restore volume deficits. For example, areas of the face that are relatively immobile, such as the top half of the nasolabial fold or the tear trough can be filled with relative ease using one of these basic injection techniques. A good correction can usually be achieved. However, none of these 4 injection patterns are satisfactory for correcting the problem of dynamic folds, such as the lower half of the nasolabial fold, the corners of the mouth, and the smile lines found in the lower lateral cheeks. These folds are quite mobile with expressive facial movements, and they present a greater challenge in providing optimal correction.

The Dynamic Nasolabial Fold
The dynamic nasolabial fold is often filled using a linear threading technique. Unfortunately, during animation there remains a tendency of the nasolabial fold to shift alongside the filler if the correction used was only a linear threading technique, which can then be easily recognized by an observer. In order to solve this problem, it is necessary to produce a broad plane across the nasolabial fold to prevent it from reappearing lateral to the filler during a smile. Many injectors attempt to use the cross-hatching technique to provide more support next to the dynamic fold, which frequently gives the appearance of an area that is overfilled at rest.
Among the most difficult wrinkles to adequately fill are the “smile lines,” which appear on the lower lateral cheek while smiling. Linear threading is usually unsatisfactory, especially during animation, and cross-hatching often results in a “pad” of filler in patients who have thin skin. In addition, even linear threading can be visible in an individual with thin skin.

The Fern Pattern Technique
To achieve correction that is undetectable at rest and during animation of a dynamic fold, Dr. Vafa uses HA not as a filler, but rather as a skin stiffening agent to attempt to increase the stiffness of the skin on either side of the dynamic line. Patients observe increased stiffness of the skin on either side of the dynamic fold after injection and a decreased ability for movement of lines in the treated area.

Fern Pattern Technique:

1.       Leaves less visible dynamic skin folds and creases – which occur during facial motion – compared to traditional pattern injections.

2.       Less product is used per area, so one syringe can correct an area which needed two syringes with traditional pattern injections

3.       We have found that the HA lasts significantly longer because of the placement technique and location of the HA in the dermal plane. Most patients come back for re-treatment in a year.

Note:

1.       The technique requires multiple small injections; therefore before the procedure we apply a topical anesthetic to the treatment area for 30 minutes, which significantly minimizes any discomfort.

Exfoliation is a must as we age and nowadays there are many options for every skin type.  Microdermabrasion is a treatment option that effectively polishes the skin, partially or completely, by removing the surface skin cells in a gradual and controlled manner using a light abrasion.  It can effectively work to remove the outer layers of sun-damaged skin, refine fine lines and wrinkles, treat blackheads and whiteheads, remove or lessen the appearance of scars and dark spots, and effectively stimulate cellular regeneration.  Microdermabrasion can also be used for treatment of acne prone skin, but must be repeated every so often to be effective. The most common areas treated are the face, neck, décolleté, hands and shoulders, but nearly any area of the body can be treated.  During the procedure, special attention is often paid to key problem areas and readies the skin for the introduction of specialized serums and other actives to the skin.  Microdermabrasion should not be used on individuals who have taken the medication Accutane in the past 6 months to a year since this product causes the skin to become thin and vulnerable.

Microdermabrasion methods include mechanical abrasion using a low-pressure suction pump that blasts tiny crystals from pen-like device in a controlled stream of micronized aluminum oxide crystals.  The used crystals and exfoliated skin cells are then sucked away by the vacuum action leaving the skin instantly softened and new.  Another method is with a diamond tip; the appropriate tip or head is attached to a wand and then manipulated over to the skin combining mild abrasion with suction to remove the dead, outer layer of skin. Neither process is particularly painful, and may result in only mild discomfort and irritation

Add-on’s such as IPL treatments and chemical peels can go a long way to further promote cellular regeneration, and circulation for complete skin wellness.  One can have spectacular results with specialized services of this kind that aggressively tackle age-related skin concerns like superficial age spots, as well as hyperpigmentation, acneic skin, oily, dry or patchy skin types and conditions,  or skin that is congested and lackluster in appearance.  A cocktail of treatments including IPL, microdermabrasion, chemical peels and facials can be alternated over a period of 3-6 weeks.  Furthermore, these other facial treatments will be enhanced by one another and the skin will become visibly fresher and renewed in a much shorter time period.

Soon after a superficial microdermabrasion treatment, you may experience mild discomfort and your skin may look pink, much like having slight sunburn, but you will still be able to return to your regular activities that same day with a few exceptions.  It is advised that you omit swimming or use of excessive heat, sunbathing (which is not recommended by skin care experts, period) or the use a tanning bed for as long as 48-72 hours following the procedure.

Also, the appropriate post-treatment home care is essential to preventing sun-induced hyperpigmentation and other damage.  It is critical that unprotected sun exposure be avoided and a broad spectrum sun protection lotion or cream be worn at all times during the day.  The sun protectant should have and an SPF of at least 15-30, and should be worn even in the winter.  If make-up must be applied, begin with a light mineral powder and avoid the use heavy foundation for up to three days, until the new skin restores its protective barrier or acid mantle.

As we continue treatments aggressively on Natalie, she sees great progress. This week we increased the RF to level 4 (of 4) for the first 2 phases of her treatment. She did not complain about any sensitivity or bruising as a result. She walked into her appointment and said that she felt her jeans were fitting better and that she felt much better overall.

As we were taking her measurements we learned that she lost another ½ inch from the circumference of each thigh this week!! Jeans don’t lie!!

These are the before and after pictures of Natalia. The results are noticeable after 4 treatments with 2 to 4 treatments more to go.

Lisa Treatment #4

Lisa really looks forward to her weekly treatments at Lasertouch Soho. “It is a treatment that has changed my life” she was heard saying last time we saw her!  She feels great during and after the treatments and is happy to report that she has almost no cellulite left in the areas that were treated. During her consultation, Lisa was asked what she looks forward to gaining most from these treatments and she answered “A confidence boost and a much-improved body image.” And it seems she’s getting what she asked for and we can tell.  The technicians continue to work with vacuum level 4 (of 4) as she enjoys the massage like quality of the treatment.

Susan Treatment #4

Susan has seen wonderful results from her treatments. She’s noticed a significant reduction in the amount of cellulite in the backs of her thighs and buttocks and great improvement in the quality of her skin texture in that area. At the end of her treatment Susan said, “I feel great and my legs look better than ever!”  We continue to treat her with RF level 3 for the entirety of her session and vacuum level 4 for the first half of her treatments. Susan has not complained about any sensitivity or bruising as an effect of her treatments.

Skin tags do not usually grow or change and can crop up almost anywhere. The cause of these growths is unclear, but new research shows a relationship between skin tags and increased insulin levels. Cutaneous tags are most commonly found among those who are overweight, insulin resistant, have pre-diabetes or are diabetic.

By decreasing your sugar intake, research suggests the reoccurrence of skin tags can dramatically reduced or even prevented. A diet of mostly low-glycemic foods such as apples, berries, brown rice, multigrain cereals and breads, and vegetables is recommended. As it happens, when we eat foods high in carbohydrates, such as baked goods, pastries, white breads and rice, the insulin in our body increases in an effort to take away the sugar from the blood stream. The sugar is then deposited in fat cells and when it is not metabolized successfully, the insulin level raises dramatically causing impaired carbohydrate metabolism, or insulin resistance. By decreasing the intake of these high-glycemic foods our body’s need for high amounts of insulin is reduced.

You may not be able to resolve existing skin tags by moderating your sugar intake, but you can have them effectively and painlessly removed with radiosurgery. Radiosurgery is a medical, non-invasive treatment of benign and malignant skin lesions. There is usually no re-growth or scar formation after a skin tag is removed and within seven to ten days the treated area heals leaving new skin which gradually blends into the surrounding tissue. From time to time, a biopsy may be performed to diagnose an atypical looking skin tag, but usually the doctor’s analysis can be determined solely on the appearance of the growth itself.

Mini Case Study – 3rd post

Lisa Treatment #3:

Lisa is really starting to see noticeable improvements in the tightness of her skin. She enjoys the treatments and does not feel any discomfort during, even at the highest settings which is not for everyone. Her LaserTouch SoHo technician works aggressively to attack cellulite at its source. Natalie Treatment #3:

We are excited to report that since Natalie began treatments with Reaction, she has lost nearly ½ an inch on her thighs. That is great progress!!

For her third treatment, the technician began the session with the highest vacuum level.  This time, boosted by the positive results, her technician chose to stay at that level for the duration of her treatment. Although overall Natalie felt little discomfort, she did have some sensitivity on her right leg during the treatment.  Her technician predicted that she might have some slight bruising on that side, but after following up with Natalie, we learned that she didn’t have any post-treatment bruising at all.

Mini Case Study – 2^nd post

Lisa Treatment #2:

Lisa enjoys the Reaction treatment very much. Throughout the treatment she kept saying that it felt like a really great shiatsu massage. When the LaserTouch Soho technician asked Lisa if they could increase the suction level, Lisa welcomed the idea and enjoyed the treatment even more!!

Natalie enjoyed treatment #2 very much.  Although the technician increased the suction level for this treatment, Natalie felt much less discomfort than she did during her first treatment. She felt very calm and relaxed throughout and even dozed off for a moment. She did not notice a very big improvement since the first treatment, however, the technician explained to her that it is perfectly normal not to see results right away, which put her at ease.

Viora and LaserTouch Soho are conducting a mini case study following the progress of four women’s six-week treatment regimen to reduce cellulite with Viora’s Reaction system. The women’s age and fitness level varies.  We will post her results and observations to the revolutionary new Reaction™ aesthetic system.

Natalie

Natalie is in her early twenties and her weight has caused her suffering for much of her life. Recently, she began developing cellulite on her buttocks and the back of her thighs. She was approached by Lasertouch Soho to participate in this case study. We will track her results during the six-week treatment.

For the majority of the time during her first treatment, Natalie claims to have really enjoyed the treatment, comparing it to a deep tissue massage. There were points where the heat became a little intense which brought about minimal pain which was easily tolerable.  However, for the most part she did not find the process painful because the aesthetician tailored the treatment to her comfort level.

During her consultation, Natalie expressed concern over her tendency to bruise easily, however she did not experience any bruising or unpleasant side effects as a result of the treatment.  Good Luck, we can’t wait to see your progress!!

Lisa:

Lisa is in her late twenties. She is in fairly good shape and is at a healthy weight for her height and frame. Although Lisa is moderately active she suffers from small pockets of cellulite on the backs of her thighs.

Lisa enjoyed the cellulite treatment very much and even commented that she would love to take the machine home with her. She found the process very therapeutic and relaxing.

Lisa did not complain about any pain or bruising following the treatment. She is looking forward to her next session and we look forward to her progress.
